New Enrolling Kindergarten, 1st ,or 2nd grade student
Required Documents
Items needed to register your child are:
- Enrollment forms
- State certified birth certificate (Must be original)- A copy for student’s records will be made.
- Form 121 (Must be original)
- Student’s Social Security Card - A copy will be made for student’s records
- Proof of Residency
Elementary Enrollment Forms 1st & 2nd
You may be required to fill out additional forms at the school.
