Steen's Creek Transportation Change Form
Please fill out the transportation form and send with your child.
All transportation changes must be in the form of a written note and sent daily/weekly to your child’s teacher. For the safety of students, we cannot take a transportation change over the telephone, by email, or text. You may send it with your child in the planner, bring it to the school office, or send a fax to 601-893-0110 by 12:30 p.m.
The written note must include: date written, full name of student, teacher’s name, the address of the bus change or name of the adult picking up the student in the car rider line, signature of the parent or guardian, and phone number where the parent or guardian can be reached.
