Please fill out the transportation form and send with your child. 

All transportation changes must be in the form of a written note. For the safety of students, we cannot take a transportation change over the telephone. The written note must include: date written, full name of student, teacher’s name, the address of the bus change or name of adult picking up a student in the car rider line, signature of the parent or guardian, and phone number where parent or guardian can be reached.
