New Student Registration Information

 

 

New Enrolling Kindergarten, 1st ,or 2nd grade student 

Required Documents

Items needed to register your child are:

•  Enrollment forms 
•  State certified birth certificate (Must be original)- A copy for student’s records will be made.
• Form 121 (Must be original)
•  Student’s Social Security Card - A copy will be made for student’s records
•  Proof of Residency

You may be required to fill out additional forms at the school.
